Compassus’ SNF at Home program is a post-acute care model designed to allow more patients to recover at home. We provide patients with comprehensive clinical care and non-clinical support from referral to discharge. We comprehensively assess the patient’s needs, preferences and goals, and tailor our services to ensure the best possible outcomes.
SNF at Home enables patients to effectively recover and break the cycle of recurrent rehospitalizations. Through collaborative clinical and ancillary care coordination, seamless transitions to the right level of care and innovation in home recovery solutions, patients are able to recover wherever they call home while meeting all their care goals.
