June 9, 2023 – Jen Hale, VP of clinical quality & standards, was featured on the recent CHAPcast episode to discuss the application of Age-Friendly Care in hospice. The Compassus – New Orleans hospice program is the first hospice provider in the country to earn the Age-Friendly Care at Home certification, which is no small task given the accrediting organization’s rigorous standards.
The CHAP evaluation focuses on a set of four evidence-based elements established by the Age-Friendly Health System movement known as the 4Ms: What Matters, Medication, Mentation and Mobility.
In the episode, Jen discusses how Compassus embraced the 4Ms and shifted to a hospice-focused approach to provide patients with comfort, safety and quality of life. Using the 4Ms, Compassus developed our internal Care Delivery Model framework, to provide standardized care in an individualized way.
