Compassionate Care Reflects the Heart of Ascension at Home Together with Compassus

A patient under the care of Ascension at Home shared a reflection on two team members: Holly, a registered nurse, and Matt, a physical therapist. Holly’s clinical expertise and attentive approach offered reassurance when the patient felt uncertain about their needs. She worked closely to assess and address those needs, creating a care plan and bringing clarity to a challenging journey. 

On the physical therapy side, Matt’s steady encouragement helped the patient make significant strides. With a calm and supportive presence, he provided solutions and motivated them to overcome self-doubt and pursue meaningful progress.
